The gap between what specialists know and what patients understand is wide, and it’s filled on one side by overclaiming skincare marketing and on the other by genuinely useful clinical guidance that most people never encounter because it lives in journals rather than in the places people actually look for information.
What follows is a distillation of the current expert consensus on some of the most common skin concerns, in language that’s useful for someone trying to make informed decisions rather than for someone preparing for a board exam.
On Acne: Treat It Earlier Than Feels Necessary
Dermatologists are broadly aligned on one point that runs counter to the wait-and-see approach many patients and GPs take with acne: early, effective treatment yields better long-term outcomes than managing it minimally and hoping it resolves on its own.
The reason is scarring. Acne that’s allowed to progress without adequate treatment damages the dermis and leaves permanent structural changes. Those changes are far harder to address than the acne itself. The consensus recommendation is that acne causing any distress, whether through severity, persistence, or the marks it leaves, warrants treatment adequate to control it rather than the mildest possible intervention that produces only partial improvement.
First-line treatments recommended by specialists include topical retinoids and benzoyl peroxide for mild to moderate acne, with oral antibiotics, hormonal treatments, or isotretinoin for more severe or treatment-resistant cases. The question of which is appropriate depends on the presentation, and it’s a conversation worth having with a dermatologist rather than a GP for anything beyond mild breakouts.
On Hyperpigmentation: Sun Protection First, Always
Experts are so consistent about hyperpigmentation that it’s repetitive, because the message doesn’t seem to land reliably: every treatment for pigmentation is undermined by inadequate sun protection. UV exposure triggers melanin production, so any brightening treatment applied without daily SPF is attempting to address a problem while the primary cause goes unchecked.
The most evidence-supported topical ingredients for hyperpigmentation are retinoids, vitamin C, niacinamide, and azelaic acid, which should be used consistently over months rather than weeks. Hydroquinone, available on prescription in the UK, is more potent and used for stubborn cases. Melasma in particular is one of the more challenging pigmentation concerns: it responds to treatment but recurs with sun exposure and hormonal changes, which means management is long-term rather than a fixed course.
For pigmentation that hasn’t responded to a well-designed topical regimen with consistent sun protection, professional treatments, including chemical peels, laser, and light-based therapies, offer more targeted intervention. The choice depends on skin tone as much as pigmentation type, since some energy-based treatments carry a higher risk of post-inflammatory hyperpigmentation in darker skin tones.
On Scar Treatment: Match the Treatment to the Scar
The clearest consensus in expert discussions of scar treatment is that scar type determines the appropriate intervention, and that treating scars without first categorising them leads to worse outcomes.
Atrophic scars (pitted, sunken, most commonly from acne or chickenpox) involve tissue deficit beneath the surface. Topical products don’t address this. Treatments that stimulate collagen production in the dermis, including microneedling, fractional laser, and subcision for tethered scars, are the appropriate clinical response.
Hypertrophic scars (raised, within the wound boundary) and keloids (raised, extending beyond the original wound and continuing to grow) represent the opposite problem: excessive collagen production rather than deficit. Silicone therapy, pressure treatment, corticosteroid injections, and, in more complex cases, laser or surgical intervention address the excess rather than stimulating more production.

The corollary is that waiting doesn’t improve the odds, and for many patients, the delay between noticing a scar and seeking treatment often extends far beyond the optimal window, with no clinical benefit.
On Anti-Ageing: Retinoids and Sunscreen, Then Everything Else
The dermatologist consensus on anti-ageing skincare is remarkably unified, given how cluttered the market is. Retinoids and daily broad-spectrum sun protection have the strongest evidence base by a significant margin. Everything else is either adjunctive or unproven.
This doesn’t mean other ingredients have no value. Vitamin C, peptides, hyaluronic acid, and niacinamide are all supported by varying degrees of evidence and have roles in a well-rounded skincare routine. But the claims made in most of the premium skincare market are not supported by clinical evidence to the same extent as retinoids and SPF.
Professional treatments, including laser resurfacing, radiofrequency, and injectable treatments (botulinum toxin and dermal fillers), address structural changes that topical products cannot reach. Experts broadly recommend establishing a strong skincare foundation before adding professional treatments, both because the foundation makes treatments more effective and because good skincare extends the results of professional intervention.
